Download and print the UESRTC Class Pre-Registration Form (PDF)
Open the file with Adobe Acrobat Reader and then select 'Print' from the 'File' menu. It should print in 'landscape' mode by default. After the form has been completed, fax it to the UESRTC at (410) 758-3573.
